The NDIS supports people with a life-long and significant disability through an individualised plan. The NDIS Transition Officer will be responsible for engaging and building relationships with carers of people expected to be eligible for an NDIS package.
The successful incumbent will be required to guide the person, and their support network, to assist with their application in order to obtain an NDIS plan.
Developing relationships with various stakeholders, engagement and assessment of clients and coordination of projects and events are some of the responsibilities involved in the position of NDIS Transition Officer.
Who we are looking for:
The successful candidate will possess a strong understanding of the NDIS and will be required to support people who have expressed interest in receiving HelpingMinds support to be engaged and effectively transitioned to suitable services.
To be considered for the role, the candidate will have a good understanding of mental health issues and the proven ability to develop positive working relationships with community groups. A tertiary qualification in the field of disability services or mental health will also be required.
