You currently receive a service from HelpingMinds which may be funded by the Mental Health Commission to provide you with the support you may need to progress your recovery journey.
The Mental Health Commission carries out routine evaluations of all its funded services. As part of this process, the Commission has arranged an evaluation of this organisation to assess how services are continuously improving to support you to meet your individual goals and needs, as well as meeting the National Standards for Mental Health Services.
This evaluation will be conducted by an independent Evaluation Team that would like to talk to as many people as possible about their experience of our services.
We encourage you to participate in this process, it is voluntary and confidential; the Mental Health Commission will not be informed of who has participated in the evaluation.
The Evaluation Team would like to know things like:
- Are the services or supports you get meeting your needs?
- Are the supports you get helping you to gain skills and confidence?
- Are the supports you get helping you to enjoy a better quality of life?
Even if you agree to participate you can choose to withdraw from the evaluation at any time without giving a reason. You can do this by letting the Evaluation Team or the organisation know at any time.
If you are happy to provide feedback on the quality of the services you receive the Evaluation Team would like to be able to contact you directly to make a time to speak with you.
Before this can happen your written consent is needed. The Consent Form asks you to allow HelpingMinds to provide your name and contact details to the Evaluation Team so that an Evaluator can contact you directly.
The Consent Form also asks if you are happy for the Evaluator to review your Individual Plan or personal files held by the organisation. This is to make sure they are keeping track of the supports provided to you to help you on your recovery journey.
All the feedback you provide to the Evaluation Team about the way your services are provided, will be kept confidential. All the information that the organisation shares with the Evaluation Team about the services it provides to you will also be kept confidential.
At the end of the Quality Evaluation process, the Evaluation Team will write a report on how well the organisation is performing against the Mental Health Outcomes and the National Standards for Mental Health Services. The report will NOT identify individual consumers, families/carers or staff.
If you are happy for the Evaluator to be given your name and contact details and for the organisation to share your Individual Plan and /or personal files with the Evaluator, then please complete the return information in the survey link.
Alternatively you can email the organisation on MHCQE2017@HelpingMinds.org.au and say “Yes I would like to participate in the Quality Evaluation” and “Yes I consent to providing access to my Individual Plan / personal files”.
You can also contact the Evaluation Team directly if you wish, and you can remain completely anonymous. If you contact the Team Leader of the Evaluation Team directly you will be consenting to providing feedback only. You may be asked if you are interested in signing the consent form.
